Our licensed Medical Social Workers are compassionate and knowledgeable about the local and state level community resources. This allows them to be effective advocates for the patient and his/her family. With this solid professional background and expertise in this field, they provide valuable resources to help improve the patients’ quality of life. The following are some of the services through our Medical Social Services:
- Assess and identify resources that can help the patient with personal, emotional and financial difficulties
- Provide information about community resources that are beneficial for the patient
- Provide counseling services and recommend appropriate treatment programs for the patient
- Assess/educate the patient and family members about how to handle the health related challenges
- Assist with helping the patient reach independence or find a long-term placement facilityÂ
Where applicable, our Medical Social Workers will work closely with other allied professionals to meet the needs of the patient.
