You or your loved one can request for Home Health Care Services if:
- You are under the care of a Physician
- You have a Plan of Care developed with you and your Physician
- You have a difficulty leaving your home unassisted
- You are in need of temporary or permanent skilled medical services.
How much will it cost the patient?
- All services for Medicare beneficiaries will be 100% covered under the Medicare Program. This means that there is NO OUT OF POCKET COST to YOU, or YOUR FAMILY, if all the required Medicare conditions are satisfactorily met.
- Sensory Impaired Patients will be provided with a qualified sign language interpreter and/or other auxiliary aides at NO COST, as required
